The Habilitation Specialist 1 is needed to provide clinical oversight and habilitation services to individuals receiving program services.
Primary Duties and Responsibilities
Duties include, but are not limited to:
o Provide habilitation services to assigned persons with intellectual and other developmental disabilities in a manner reflecting the stated values of OPWDD:
Compassion, Dignity, Diversity, Excellence and Honesty. o Develop written habilitation plans that address independence, integration, individualization and productivity and the outcomes as reflected in the Lifeplan (LP), as well as the needs of the individuals. o Develop and maintain Day Hab-specific and individual-specific Staff Action Plans o Provide guidance to all direct-care staff in the program area who are responsible for employing proper strategies with the individuals that are consistent with their habilitation plans o Ensure all Day Habilitation documents are neatly maintained, per regulation and policy, and are filed in the individual's records, in proper order o Provide input to the Day Hab Manager and /or Recreation staff in planning of community inclusion activities that are appropriate to each individuals' interest and abilities o Ensure the Day Habilitation billing is completed, verified for accuracy and submitted on a daily basis o Attend all treatment related and team level meetings including, but not limited to CFS's, LP's and quarterly reviews. o Other tasks as assigned by supervisor.

Salary Range:
From $45455 to $58028 Annually

Qualifications:
Must have QIDP certification/status and permanent service as a Habilitation Specialist 1 OR must be active and reachable on the current Civil Service eligible list for the Habilitation Specialist 1. Transfer
Qualifications:
Candidates must have QIDP certification/status and be eligible for transfer under Section 70.1 of the Civil Service Law. Provisional
Qualifications:
Candidates must be eligible for and have registered to take the Civil Service Examination to be given on 12/14/19. OPERATIONAL PREFERENCE:
QIDP (Qualified Intellectual Disorder Professional) designation requires one year's experience working with people with a developmental disability AND a bachelor's degree or higher in a human services field (including, but not limited to:
sociology, special education, rehabilitation counseling or psychology).Estimated Salary: $20 to $28 per hour based on qualifications.
